• Initial Curiosity Call (20-minutes): FREE

• Individual Sessions (55-minutes): $150*

• Couple's & Family Sessions (60-minutes): $160

• Parenting Group sessions (90-minutes): $50 

• Late cancellation/Missed Appointment Fee: $100

INSURANCE 

I am private pay and am not in-network with insurance companies. I am happy to provide an itemized receipt of services (called a "Superbill") for you to submit to your insurance provider for possible reimbursement of out-of-network services. Please contact your insurance company directly to learn the specifics of if/what your insurance policy may reimburse for services received from an out-of-network mental health provider.

Information on the "No Surprises Act"

In compliance with the No Surprises Act, all healthcare providers  are required to notify clients of their Federal rights and protections against “surprise billing.” 

This Act requires that we notify you of your federally protected rights to receive a notification when  services are rendered by an out-of-network provider, if a client is uninsured, or if a client elects not to use their insurance. 

Additionally, we are required to provide you with a Good Faith Estimate of the cost of services (attached).  It is difficult to determine the true length of treatment for mental health care, and each client has a right to decide how long they would like to participate in mental health care. Therefore, attached you will find a fee schedule for the services typically offered by your therapist, and we will collaborate with you on a regular basis to determine how many sessions you may need.  

It is a Federal requirement that we have each client sign this form to begin/resume treatment. If you choose to obtain treatment with us, please sign, date, and return before your next appointment.  If you have any questions, please don’t hesitate to ask.

CANCELLATION POLICY 

In order for progress to be made in therapy, it is critical that scheduled sessions are held consistently.  If an appointment cannot be kept, at least 24-hours advanced notice is required for any appointment cancellations.  This gives us the best chance to reschedule during the same week.  Other than in emergency situations, a cancellation fee ($100) will be charged for any late cancellations or missed appointments.

* Please note that late cancellation and missed appointment fees are not eligible for reimbursement from insurance companies and are not considered eligible expenses for HSAs or FSAs.
